We understand that any medical office visit or procedure can be stressful, but planning ahead for the little details will help relieve some of that anxiety and help you get the most out of your time with the surgeon.  Below are some topics we think you will find useful:

Surgery Forms

There are a few forms that are specific to our surgery center that you will need to complete.  We suggest that you download these forms to fill out at home and then bring the completed forms with you on the day of your surgery.

Patient Records

Your records are automatically transferred from your doctor’s office to East Valley Outpatient Surgery.

East Valley Outpatient Surgery is committed to help you understand your health insurance policy and how it works in relationship with your upcoming surgery. We have included basic explanations of the terms within your health insurance policy and your financial obligations  so that you can make informed health care decisions.

After your surgery you will receive a statement called an Explanation of Benefits (EOB) from your insurance company.  It itemizes the fees charged and indicates the allowable amount covered by your insurance company.  After receiving the EOB, you will receive bills from East Valley Outpatient Surgery, your surgeon, and your anesthesiologist for your rendered care.

Some insurance carriers refer to East Valley Outpatient Surgery as an out-of-network facility. This does not mean that we do not accept your insurance, but rather, it means that we may not currently have a contract with your health insurance provider.

The Patients’ Rights & Responsibilities (or Bill of Rights) is a document that provides information on how both patients and medical staff can reasonably expect to be treated during the course of medical care. While such documents are not legally-binding, they do provide goals and expectations for establishing a positive patient and facility relationship.
